It's Easy
All you need to join is a completed application form, payment,
and proof of licensure (or educational status) and insurance, if
applicable. Use this easy checklist to walk you
through the steps.


Applications

You have several options:

Fastest: Click here to open an email to Gaylesta. Include your
full name, license number or intern registration. Within 24 hours, you will receive an email with a log-in ID and temporary password. You will then be able to access the Members Only section of this site. There you can complete all necessary information online, and submit your completed application.

Click here to access a PDF form, which you can save to your desktop, complete, and send as an email attachment. When completed, click here and attach the PDF form to the email. Or you can mail the completed forms to the address in the left column.

Call us toll-free at 877-433-9939 to request that an application form be sent to you.

Proof of licensure or educational status
Choose one of these options:

To become a clinical member, you must provide proof of licensure, either via mail
or by scanning it and emailing us an electronic copy

To become a prelicensed member, you must provide a copy of your current
student ID, transcript, BBS intern registration, or other enrollment confirmation.

Submissions
Once your basic information has been added to our database (see center panel for details), applications can be easily completed online. If you have difficulties, click here to open an email to Gaylesta. Provide your name, contact information, and a brief description of your problem; you will be contacted. Other required documents can be mailed to :

Timing
Applications submitted electronically will be processed within a week, once all materials, including payment, have been confirmed. Applications received by mail will be processed as soon as they arrive in our office. Our membership year runs April to April; memberships will expire in April of the following year.

Once your application has been processed, you will be able to access your information online and get details about committee activities and upcoming events in our member section.
